I am a Canadian married to an Iranian (also for 18 years) and we made the trip 'home' in 1995. We went for 6 weeks and that wasn't nearly enough time! In Tehran itself there are so many museums, parks, shopping districts - not to mention the Bazaar-e Bozorg, which takes a week and a half to see - that an entire trip can be planned around that city alone.
Take a taxi at least once - it's a thrill ride not to be missed. We drove to the Caspian Sea, which is breathtaking (both for the roads and the scenery), but stay at a private resort if you want to swim without wearing a tent.
Shiraz is exciting and rather more European than anywhere else, and Persepolis is a must-see.
Qom is worth a stop to see the shrines, and, if you are interested in antiquities, Kashan is marvellous. I got in a little trouble there though, because you must wear a chador and (gasp!) my ankles were showing.
Isfahan is my favorite city, and there are as many things to see there as in Tehran. You must go to the teahouse under the Seeoseh-pol bridge.
My number one hint is always carry Kleenex or wet wipes; for such a clean people, their public rest-rooms are terrible! Buy a long and light raincoat here, not too formfitting, and a couple of headscarves. A cotton scarf doesn't slip off the way a polyester or silk one will. In Tehran, almost any colour is OK, but in the other cities, something more dull like brown, grey, olive, navy or black is a better choice for a coat. If you are planning any walking out of the city, take a lighter, mid-thigh loose jacket as well (although he doubtless has a thousand relatives who would be willing to loan you something). Take diapers and feminine 'stuff' with you, because the quality of Iranian ones aren't so good. Also take Pepto Bismol and Gravol, just in case. Don't forget your U.S identification, for getting back into the country. I didn't take my son's Canadian birth certificate with me and he ended up getting a visitor's visa at the Toronto airport in order to come home! Take large and crisp U.S. dollars - they won't exchange crumpled up ones.
